We were able to replicate the problem. It happens during load in certain cases (e.g. our game Decay of Logos) or on Alt+Enter in our simpler test apps.
This is a problem introduced in Unity 5.4.x. Not sure yet if it's a Unity bug. We're still investigating. Will get back to you as soon as we have ...

Are you using HDR?
Also, is it possible for you to export a unitypackage containing the LUT you're using? Or just zip the LUT texture + meta file so we can analyze the import settings and test the LUT.

We changed the debug display from 1.3 to 1.5, so you probably had the 1.3 trial and upgraded straight to 1.5. That, at least, explains the difference in debug mode. Are you running the latest Amplify Motion version? (1.5.0)

Unfortunately we can't guarantee AC to work on the free version of Unity 4. To run in Unity 4 and earlier versions, AC requires Unity Pro features in order to work properly. You can expect it to work on Unity 5 Personal, however.
